Benefit Management’s online provider directory is the easiest way to locate current providers in your area. And because it is updated nightly, it is also an excellent resource for your provider’s current address and phone number.

Check the front of your ID card for the name of your provider network, and click on the corresponding name from the list of provider directories shown on the left. Please note that Access Care and Access Care PPO are two separate provider networks. Be careful to click on the correct network name to ensure you locate providers that are a part of your health plan.

The listing of providers in this site does not constitute a recommendation of any hospital, physician, professional provider or facility. However, certain participant contracts require the use of contracted providers to obtain benefits or the highest level of benefits. Facility-based physicians may not be contracted health care providers.

Remember: You are responsible for receiving pre-certification/prior authorization where required.
